For individuals with high income, Medicare may assign you income surcharges(IRMAA) based on your MAGI(modified adjusted gross income). The surcharges are additional premiums to your part B $185(2025) monthly premium.

Income surcharges are assessed on an annual basis based on your tax return from two years prior. For 2025, Medicare will evaluate based on your 2023 tax return.

If you experience a life-changing event(such as marriage, divorce, death of your spouse, work stoppage/reduction, etc), you may be able to reduce the income surcharges by filing an SSA-44 appeal with your local social security office. The appeal can take up to 2 months to be processed and will need to be refiled each year that you are assigned income surcharges.
